Submitted fundraisers for the '25-'26 season!

Tournament Registration Form

This form must be completed in its entirety and submitted to the LV Phantoms Youth Treasurer for processing in order for your team to attend a tournament. Please allow one week for processing. The treasurer will contact you upon processing of the order.

In Season Roster Change Form

The deadline for in season roster change submissions is every Thursday by 11:59pm

USA Hockey Patch Submissions

Please use this form to submit your scoresheets for any players who earn a patch from USA Hockey throughout the season.
